LifeStream, Incorporated: Community Residential Supports

Our community residential services include 2 levels of support to facilitate autonomous living for adults with a primary diagnosis of mental retardation; fully furnished homes for independent living as well as individual and family supports within private homes. Our programs are individualized and provide a range of support ranging from very little in-home assistance to more comprehensive services for medically fragile individuals.

RESIDENTIAL SERVICES

Residences are designed to facilitate living within communities. LifeStream has over 30 group residences throughout southeastern Massachusetts. Each home offers appropriate support and supervision 24 hours per day.

Residential Services provide:

individualized, needs-based programs
integrated clinical services in everyday living activities
focus on home ownership, independence, everyday living activities nursing care
certified staff providing medication administration and management
24 hour medical, physical, and cognitive assistance
preventative/routine healthcare and dental assistance through transportation accessibility, appointment scheduling, etc.
barrier-free home accessibility
community interaction and social activities
ongoing needs analysis to adapt to changing physical, cognitive, and medical needs

INDEPENDENT LIVING AND INDIVIDUAL SUPPORT SERVICES

The organization supports 45 individuals living on their own, with family members, or transitioning into independent living. LifeStream offers support as needed per individual.
The program is based out of Fall River, Massachusetts and funded by the Massachusetts Department of Mental Retardation.

Independent Living and Individual Support Services provide:

individually tailored program to meet individual and family needs
living skills for next step of independence
personal advocacy training
money management and skills training
on-call support
wide ranging assistance to parents and family members with adult children
medical assistance including follow-up appointments, medication administration reminders, etc.
ongoing needs analysis to adapt to changing physical, cognitive, residential, and medical needs
