LifeStream, Incorporated: Day Support Services
DAY HABILITATION
LifeStream's Day Habilitation programs provide Medicaid funded support services for adults with disabilities residing throughout Southeastern Massachusetts. Each center provides an individually tailored, goal oriented program to meet the often-challenging needs of people requiring enhanced assistance and support. In addition to experienced and trained staff members, nursing services and therapy specialists are available at each program location to evaluate and provide clinical supports.
Since 1981 the organization has successfully provided individualized medical, therapeutic, and developmental training services to meet the needs of challenged adults. The goals of these services are to maintain and enhance personal health as well as foster the greatest degree of personal independence possible for each person. Each consumer program is individualized for each participant.

SUPPORTED EMPLOYMENT
This is a work-oriented program.
The goal of the program is to help adults with disabilities find and keep
a job. This work may be within the organization or in businesses within
local community. The program offers vocational assessment, job readiness,
and work skills training. LifeStream's Supported Employment is a "no fee"
program funded by the Commonwealth of Massachusetts. Other details:

OFF-SITE SPECIALIZED SERVICES
Off-Site Services, also referred to as Specialized Services, enables individuals diagnosed with Mental Retardation or Developmental Disabilities and who live in area nursing homes to receive all services available to center-based program participants without having to attend one of the organization's Day Habilitation centers.
A team of clinicians, therapists,
and personal advocates are mobilized from the day habilitation program to
all individuals meeting eligibility requirements. Individuals who are able
to
leave thenursing facility for an extended period of time have the option
to receive transportation services and participate in a Day Habilitation
center on a permanent basis.
We also offer training to nursing home staff in order to integrate the services
off-site and ease transition into our day programs.
REFERRALS
Referrals to LifeStream's Day Habilitation programs are generally made by the Department of Mental Retardation.
