LifeStream, Incorporated: Clinical Services

The Clinical Services Department provides quality assurance and unique clinical services at each disabilities program location in order to support optimal health of individuals served while promoting quality of life. We are focused on the highest quality of care for each consumer. All individuals are evaluated to provide services that will enable them to achieve more independent living within the community. Combined, our resources make LifeStream a leader in the field.

CLINICAL AND INSTRUCTIONAL SERVICES

At each of our Day programs, the Clinical Services Department provides specific services for participants. These services are available on-site at our day programs and off-site at
nursing homes and LifeStream's residences.

Clinical Services

• occupational: adaptive equipment, sensory motor development
• physical: increasing endurance, gross motor skills, stretching
• speech: communication, sign language, equipment assisted communication

Psychological Services

• relaxation training
• progressive muscle relaxation
• passive relaxation techniques
• Instructional Services
• functional training activities
• meeting goals in naturally occurring environments
• program specific training
• behavioral programs

This is just a sampling of the clinical and instructional resources available through LifeStream. Each program is individually tailored to meet the needs of the individual. For more

QUALITY ASSURANCE

At LifeStream, we are focused on the highest quality of care for each consumer. One of the ways we achieve that commitment is through our unique Quality Assurance program. Our program:

• Ensures each disabilities program has a trained human rights officer
• Utilizes teachable moments and natural opportunities in human rights consumer training
• Provides individual supports program
• Encourages self-advocacy
• Oversees the Medical Administration Program (MAP) in homes
• Oversees licensing and accreditation compliance for funding sources

In addition, we have three committees focused on monitoring and reviewing quality assurance issues:

• Human Rights Committee: the committee meets once per month and includes committee members and officers from each disabilities program. The purpose of the committee is to assist the provider to affirm, promote, safeguard and protect the dignity of all individuals supported by LifeStream, Inc.
  

• Peer Review Committee: the committee meets once per quarter to review behavior modification treatment plans for compliance with state guidelines.
  

• Interdisciplinary Team: the IDT team includes all disciplines and meets once per month to review the needs and services provided to all individuals supported by LifeStream.
