LifeStream, Incorporated: Clinical Services
The Clinical Services Department provides quality assurance and unique clinical services at each disabilities program location in order to support optimal health of individuals served while promoting quality of life. We are focused on the highest quality of care for each consumer. All individuals are evaluated to provide services that will enable them to achieve more independent living within the community. Combined, our resources make LifeStream a leader in the field.

CLINICAL AND INSTRUCTIONAL SERVICES
At each of our Day programs,
the Clinical Services Department provides specific services for participants.
These services are available on-site at our day programs and off-site at
nursing homes and LifeStream's residences.

QUALITY ASSURANCE
At LifeStream, we are focused on the highest quality of care for each consumer. One of the ways we achieve that commitment is through our unique Quality Assurance program. Our program:
In addition, we have three committees focused on monitoring and reviewing quality assurance issues:
Human Rights Committee:
the committee meets once per month and includes committee members and
officers from each disabilities program. The purpose of the committee
is to assist the provider to affirm, promote, safeguard and protect
the dignity of all individuals supported by LifeStream, Inc.
Peer Review Committee:
the committee meets once per quarter to review behavior modification
treatment plans for compliance with state guidelines.
Interdisciplinary Team: the IDT team includes all disciplines and meets once per month to review the needs and services provided to all individuals supported by LifeStream.
