Job
Summary:
Responsible
for ensuring the delivery of quality health care for individuals supported
in agency programs. Coordinates health care and health maintenance,
providing in-service to staff, and being a medical resource to them
as well. The RN will encourage and facilitate participant choice and
support those choices using all available resources. Reports to Nursing
Coordinator.
Key
Tasks and Responsibilities:
- Assess all individual's
medical regimes to ensure that all individuals are referred for and
receive health services, which will maximize health and safety.
- Ensures that
all Consumers are referred for and receive services which assist with
achieving set goals and maximize physical and emotional well being.
- Provide direct
care duties to maintain the highest level of care standards.
- Provides in-service
to educate staff about pertinent health issues.
- Maintenance
of medical charts, medication ordering regime and other medical supply
inventory.
- Assess all consumer
complaints and overall well-being of each individual on a daily basis.
- Assist in the
scheduling, coordinating, attending and transporting all consumers
to medical appointments as needed.
- Review doctor's
orders on a monthly basis for accuracy.
- Monitor the
general health status of all consumers on a day-to-day basis.
- Complete all
required documentation in a timely manner, including progress notes
and medical documentation.
- Serve as a liaison
between physicians and consumers and advocate for all necessary medical
services.
- Attend all mandatory
meetings and actively participate.
- Assure that
all medications are stored according to the guidelines established
by DPH.
- Adheres to DPH,
DMR, DMA Commonwealth licensure, LifeStream and LifeStream policies.
- Maintain effective
communication with peers, supervisors, parents, guardians and community
health care providers.
- Implements a
health care delivery model, which meets the diverse needs of the individuals,
served with an emphasis on consumer empowerment and use of community
resources.
- Maintains respectful
communication with all individuals served.
- Reports all
suspicions of abuse or neglect to DPPC as a mandated reporter.
- Serve as a MAP
Trainer, as requested by the agency.
Education/Experience
Requirements:
- Registered Nurse
with current licensure in Commonwealth of Massachusetts.
- Demonstrated
history of quality work performance and teamwork in previous positions.
- Familiarity
with the needs individuals with developmental disabilities.
- Excellent written
and verbal communication skills.
- Demonstrated
commitment to the philosophy of community inclusion.
