Licensed Practical Nurses for Residential Program
May 2007
LifeStream,
Inc. is seeking several part-time LPNs for its Residential Program. These positions
report to and work under the supervision of the Nurse Manager. Residential LPNs
are responsible for ensuring the delivery of quality health care for individuals
in their community residences. LPNs encourage and facilitate participant choice
and support those choices using all available resources. Part-time day and evening
hours are available and hours are flexible. Key
Tasks and Responsibilities: - Assess
all individual's medical regimes to ensure that all individuals are referred for
and receive health services, which will maximize health and safety
- Ensure
that all individuals are referred for and receive services which assist with achieving
set goals and maximize physical and emotional well being
- Provide
direct care duties to maintain the highest level of care standards
- Maintain
medical charts, medication ordering regime and other medical supply inventory
- Assess
all complaints and overall well being of each individual on a daily basis
- Review
doctor's orders for accuracy
- Adhere
to DPH regulations for medication storage, administration and destruction
- Responsible
for all medication administration and all clinical procedures
- Make
monthly notes reporting current medical status and any issues that may hinder
progress on Day Habilitation goals
- Assist
RN in obtaining reports and approval of treatment plans from primary physicians
- Make
recommendations for medical follow-up to appropriate staff and Parents/Guardians
- Monitor
the general health status of individuals on a day-to-day basis
- Attend
all mandatory meetings and actively participate
- Adhere
to DPH, DMR commonwealth licensure, LifeStream policies
- Maintain
effective communication with peers, supervisors, parents, guardians and community
health care providers
- Implement
a health care delivery model, which meets the diverse needs of the individuals,
served with an emphasis on individual empowerment and use of community resources
- Maintain
respectful communication with all individuals served
- Report
all suspicions of abuse or neglect to DPPC as a mandated reporter
Education/Experience
Requirements: - Licensed
Practical nurse with current licensure in Commonwealth of Massachusetts
- Demonstrated
history of quality work performance and teamwork in previous positions
- Excellent
written and verbal communication skills
- Demonstrated
commitment to the philosophy of community inclusion
- Familiarity
with the needs of individuals with developmental disabilities

- Valid
driver's license

of personal vehicle for work purposes
