QUALITY
ASSURANCEAt
LifeStream, we are focused on the highest quality of care for each consumer. One
of the ways we achieve that commitment is through our unique Quality Assurance
program. Our program:
- Ensures each disabilities
program has a trained human rights officer
- Utilizes
teachable moments and natural opportunities in human rights consumer training
- Provides
individual supports program
- Encourages
self-advocacy
- Oversees
the Medical Administration Program (MAP) in homes
- Oversees
licensing and accreditation compliance for funding sources
In
addition, we have three committees focused on monitoring and reviewing quality
assurance issues: - Human
Rights Committee: the committee meets once per month and includes committee
members and officers from each disabilities program. The purpose of the committee
is to assist the provider to affirm, promote, safeguard and protect the dignity
of all individuals supported by LifeStream, Inc.
- Peer
Review Committee:
the committee meets once per quarter to review behavior modification treatment
plans for compliance with state guidelines.
- Interdisciplinary
Team: the IDT team includes all disciplines and meets once per month to review
the needs and services provided to all individuals supported by LifeStream.
